This Patient Agreement is entered into between you (the customer) and UK Sleep Well when you purchase any prescription-only or controlled medication through our website, www.uksleepwell.com.
By completing a purchase, you confirm that you have read, understood, and accepted the terms set out below. If you do not agree, you must not proceed with your order.

2. Your Eligibility & Responsibilities

By placing an order, you confirm that:
